Hydroxychloroquine Plaquenil is an oral medication that is commonly used in the treatment of many types of autoimmune conditions including lichen planopilairs, frontal fibrosing alopecia, discoid lupus and pseudopelade. I’m often asked if hydroxychloroquine is an immunosuppressant or best classified as an immunomodulatory drug. I started with plaquenil last year, yes it took almost 6 months to get a level in your body, it does help, trust me. I was on Prednisone 7.5 to 10 mg per day, had a bad flair up and I am on 40 mg per day, last Friday the dr.''s told me to go down by 5 mg per week, so from 40mg to 35 mg per day. Hydroxychloroquine is ineffective in treatment of alopecia. At follow-up, 5 patients discontinued treatment because of lack of regrowth; whereas 3 patients continued hydroxychloroquine. Two patients with alopecia totalis showed minimal signs of regrowth, but relapse ensued, and hydroxychloroquine was discontinued after 148 and 197 days of treatment, respectively Fig 1. Our last patient had extensive alopecia areata and experienced widespread regrowth.
